I acknowledge that it is my duty to exercise ordinary care for the protection of myself and others while attending classes and events at Healing Realm Yoga, LLC (doing business as “Integrative Yoga Therapy”). I assume the risk of physical activity with my own physical condition. I have received advice from my doctor that I am capable of physical exercise such as instructed at Integrative Yoga Therapy, or I will seek such advice, or I will assume the risk of exercising without a doctor's examination. 

 

In the case of participation in aerial yoga at Integrative Yoga Therapy ("Activity"), I represent that I understand the nature of aerial yoga and that I am qualified, in good health, and in proper physical condition to participate in such Activities. I fully understand that such activity involve risks of serious body injury, including permanent disability, paralysis and death. I fully accept and assume all such risks and all responsibility for losses, costs and damages I incur as a result of my participation in the Activity.

 

I hereby grant Integrative Yoga Therapy permission to use my likeness in a photograph, video, or other digital media ("photo") in any and all of its publications, including web-based publications, without payment or other consideration. In consideration of the current COVID-19 (also known as Coronavirus) global pandemic, I represent that I understand the nature and safety precautions associated with practicing yoga at Integrative Yoga Therapy (“Activity") during the COVID-19 global pandemic. I state that I am qualified to practice under such circumstances in that I am in good health and proper physical condition to participate in such Activities. I fully understand that such activity involves risk of potential exposure to COVID-19 despite the best efforts of Integrative Yoga Therapy to clean, sanitize, and promote social distancing. I fully accept and assume all such risks and all responsibility for losses, costs, and damages I incur as a result of my participation in the Activity. 

I hereby release, discharge, and covenant not to sue Healing Realm Yoga, LLC "Releasees", its respective administrators, directors, agents, officers, volunteers, subcontractors and employees, other participants, any sponsors, advertisers, and, if applicable, owners and lessors of premises on which the Activity takes place from all liability, claims, demands, losses, or damages on my account caused or alleged to be caused in whole or in part by the negligence of the "releasees" or otherwise, including negligent rescue operations; and I further agree that if despite this release, waiver of liability, and assumption of risk I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save and hold harmless each of the releasees from any loss, liability, damage, or cost which any may incur as the result of such claim.
